For the Ukrainian passport for travel abroad (паспорт громадянина України для виїзду за кордон), a biometric document whose face image the State Migration Service captures at the office, so adults bring no photograph. Children under 12 and persons who cannot attend for health reasons may instead hand in one 10 x 15 cm print that is scanned. Not for the ID-card internal passport (see ua/id-card) or a Ukrainian visa.

What trips people up
- CheckYou bring no photo. The ДМС schedules each appointment for 'перевірки та оформлення документів, фотографування, сканування відбитків пальців тощо' - checking documents, photographing and fingerprinting - and the document list for the foreign passport names no photograph for adults.
- CheckThe only paper photo is the exception for children under 12 or people who cannot travel to the office: 'може бути подано одну фотокартку розміром 10 х 15 сантиметрів для внесення відцифрованого зображення особи шляхом сканування'. It is a 10 x 15 cm print, not a 35 x 45 passport photo.
- CheckFingerprints are scanned from age 12 at the same visit: 'Сканування відбитків пальців рук дітей здійснюється з 12 років.' Every child needs a separate appointment slot ('на кожну дитину потрібен окремий запис у черзі').
- CheckThe application form is printed by the clerk, not brought: 'заяву-анкету (друкується працівником, який приймає документи)'. Apply at a ДМС unit, a ЦНАП or a 'Паспортний сервіс' centre of ДП «Документ».
- CheckFees on the saved page: administrative service 352 UAH (20 working days) or 992 UAH (urgent), plus 795 UAH for the blank.

The crown-to-chin portion of the facial image shall be 70 to 80 per cent of the longest dimension defined for Zone V, maintaining the aspect ratio between the crown-to-chin and ear-to-ear details of the face of the holder.
